Care Coordination Outreach Worker

Job overview

The Care Coordination Outreach Worker role is based primarily in Park Forest, IL, and at the Chicago Heights Family Health Center, with travel to Humboldt Park approximately one or two times per month; starting pay is $17.50 per hour. Aunt Martha’s Health & Wellness works to improve community health and help patients access coordinated care. The worker conducts screenings and outreach, arranges follow-up appointments, and provides preventive health education. The role collaborates with health plans, hospitals, specialists, physicians, and clinic teams to help patients stay connected to needed services and address barriers to care.

What you'll do

• Complete health risk screenings
• conduct clinic, home, and hospital outreach as required
• schedule follow-up appointments after emergency department or inpatient discharge
• connect patients with medical services
• provide preventive health education
• collaborate with health plans and care teams
• document outreach in Athena EHR
• maintain program records and patient confidentiality
• answer and route calls
• attend meetings and training.
