What you'll do

• Lead and oversee nurses, aides, social workers, chaplains, bereavement coordinators, and volunteers
• provide real-time clinical guidance and mentoring
• lead interdisciplinary group meetings
• ensure plans of care are accurate and documented
• conduct record and case reviews and home visits as needed
• ensure Medicare hospice compliance and survey readiness
• manage staff scheduling
• update care policies and emergency preparedness procedures
• coordinate communication among patients, families, providers, and the care team
• maximize resource utilization.
