What you'll do

  • Coordinate care with team members and support compliance with care plans
  • arrange provider, PT, and OT visits
  • deliver medications and review instructions
  • reconcile medication lists
  • facilitate telehealth visits
  • train patients on remote patient monitoring equipment
  • deliver and set up durable medical equipment and train patients and caregivers
  • perform routine venipuncture and collect blood specimens as needed.
