Registered Nurse (RN) - Home Health

Utilizing the nursing process (assessment, diagnosis, outcomes/planning, implementation and evaluation) to provide professional nursing services to patients at home. Collaborating with patient, family, and other inter-professional colleagues to plan, implement, and evaluate care. Demonstrating competency and practicing within the full scope of nursing expertise/knowledge and utilizing appropriate
