Home Care Coordinator

What you'll do

• Coordinate client and caregiver schedules
• resolve scheduling concerns
• accept and coordinate referrals and intakes
• monitor service-change records
• match client authorizations with caregivers of appropriate skill levels
• confirm schedules and changes
• arrange nursing introductions and orientations
• report client status changes
• coach caregivers and escalate concerns
• support recruitment and onboarding
• provide rotating night and weekend on-call coverage as assigned.
