Hospice Registered Nurse (RN) Case Manager

What you'll do

• Assess patients and families to determine hospice needs

• administer prescribed medications and treatments
• counsel and educate patients and caregivers
• coordinate assigned caseloads and communicate with physicians and community health contacts
• maintain clinical notes
• participate in interdisciplinary meetings and quality improvement
• participate in on-call duties as defined by policy.
