What you'll do

• Assess, identify, secure, and monitor services for participants. Respond to participant needs
• complete assessments, service plans, visits, calls, and incident reporting within required timeframes. Document visits and contacts in required systems, provide training and support to participants and consumer employees, maintain internal databases, and help obtain accessibility adaptations, specialized medical equipment, and non-medical transportation. Travel throughout Cumberland and Dauphin counties and surrounding areas.
