Hospice Account Executive / Patient Care Advocate

What you'll do

• Develop referral growth in the assigned territory
• build relationships with physicians, facilities, care-at-home providers, and community agencies
• create and evaluate territory plans
• make sales calls and presentations
• meet with patients and families to discuss home-care services as needed
• maintain referral-source and competitor profiles and activity records
• coordinate communication with intake and patient-care resources
• participate in community education and marketing efforts as requested
• prepare monthly tracking reports.
