What you'll do

• Conduct alcohol and drug counseling sessions and group counseling as required
• orient patients to the program
• gather assessment information
• develop and update treatment plans
• maintain and submit patient records
• document contacts
• make referrals and follow up
• provide intake, readmission, crisis support, and discharge follow-up services
• coordinate with community providers
• attend treatment team meetings and provide training as needed.
