What you'll do

• Provide case management to an assigned patient caseload
• collaborate with multidisciplinary staff on individualized treatment plans
• conduct outreach and follow-up
• assist with vocational referrals and counseling
• coordinate discharge planning and referrals for housing, employment, medical, psychiatric, financial, and legal needs
• document contacts and progress in medical records
• attend required meetings and training.
