The Home Health Registered Nurse role is onsite in Melbourne, FL, providing in-home visits within a selected territory. Compensation is $1,500–$2,000 per week, with approximately $65–$70 for visits above the 30-visit weekly productivity threshold. Visits fall between 8:00 AM and 8:00 PM, though the nurse will not work all 12 hours. Wheatland Health Community Care’s mission is to help people live safely and independently at home. The nurse provides skilled in-home care through assessments, care planning, patient and family education, accurate documentation, and coordination with clinicians and care teams. Monitoring progress and escalating concerns helps support continuity, compliance, and patient safety.

What you'll do

• Perform in-home skilled nursing visits, including starts of care, recertifications, routine visits, and discharges
• complete assessments and implement plans of care
• educate patients and families
• document care, orders, and signatures in the EMR
• coordinate with physicians, therapists, care coordinators, and administrative staff
• monitor progress and escalate changes or safety concerns
• follow policies, payer requirements, and home-health standards
• participate in quality assurance and care-plan updates.
