What you'll do

• Remain engaged with the client
• assist with activities of daily living, bathing, grooming, toileting, mobility, and approved transfers
• support fall prevention and safety monitoring
• prepare meals and snacks and encourage hydration
• provide medication reminders only as permitted by policy and the care plan
• support activities and daily routines
• perform light housekeeping and client laundry
• observe and report changes or incidents
• complete timely WellSky documentation
• communicate with operations and provide shift handoff when applicable
• remain for the full scheduled shift.
