What you'll do

• Gather patient health histories
• conduct follow-up calls
• discuss test results, referrals, medication changes, and care plans
• triage walk-in and phone patients
• coordinate care activities
• maintain patient panels and clinical measures
• lead multidisciplinary performance-improvement efforts
• provide emergency care as needed
• conduct nurse visits and treatments
• administer prescribed medications
• document patient encounters
• coordinate pre-visit planning.
