What you'll do

• Conduct MDS assessments to identify resident needs and acuity
• develop and implement individualized care plans in accordance with Medicare requirements and ethical standards
• collaborate with social services, dieticians, rehabilitation specialists, and medical staff
• monitor care plans and adjust as needed
• ensure compliance with Department of Health requirements and regulations.
