What you'll do

• Identify patients at risk of complications and coordinate treatment with patients and the Care Coordination team
• develop strategies to improve health and quality-of-life outcomes
• coordinate with primary care providers, specialists, families, community resources, and care programs
• educate patients on chronic conditions
• review charts and facilitate preventive care follow-up
• use motivational interviewing
• advocate for patient needs
• protect confidential information
• participate in training, meetings, and quality improvement
• assist with new staff orientation as needed.
