What you'll do

• Provide personal care and assist with activities of daily living according to the plan of care
• maintain a safe environment
• assist with ambulation, transfers, and exercises as directed
• prepare nutritional snacks
• report changes in patient status to the nurse
• document care and observations
• complete charting, timesheets, and mileage logs
• attend care conferences and team meetings
• and participate in scheduled on-call rotation for Home Care only.
