Remote Care Coordinator, Cardiac

Cardiac Care Alliance works with independent cardiologists to improve patient access, outcomes, and care experience through proactive coordination. The coordinator supports patients with cardiac conditions, primarily congestive heart failure, through telephonic outreach, education, follow-up, and care-gap tracking. The role works with registered nurses, cardiologists, primary care offices, and other partners, escalating concerns to licensed clinicians as appropriate.

What you'll do

• Conduct structured phone outreach
• manage an assigned patient caseload using risk stratification
• complete assessments and follow-ups
• identify social needs and connect patients with resources
• review population health dashboards and address care gaps
• provide patient education
• support post-discharge follow-up within 48 hours
• document interventions and goals
• communicate with clinical partners and escalate concerns.
