The Medical Social Worker (LSW/LGSW/LISW/LICSW) — PRN Hospice role is onsite in Buffalo, MN, serving the Minneapolis–St. Paul metro and surrounding cities. Pay is $33–$37 per hour, depending on license and experience. Lifespark supports seniors in navigating health care and living more independently as they age. This role provides hospice social work services by assessing patients’ and families’ psychosocial needs, counseling and assisting with crisis intervention, connecting them with resources, and contributing to individualized care planning with the interdisciplinary team.

What you'll do

• Assess patients’ and families’ psychosocial status and communicate findings to the RN case manager and interdisciplinary group
• counsel patients and caregivers, provide crisis intervention, maintain clinical records, assist with advance directives, connect families with community resources, and participate in care planning and quality improvement.
