What you'll do

• Pick up and deliver prepared food boxes to patient residences on assigned routes and schedules
• conduct brief wellness checks and report concerns
• maintain delivery confirmations
• follow traffic laws and company policies
• connect individuals to health care services
• provide basic health education and coaching
• assist with resource navigation and access to services for vulnerable populations.
