What you'll do

• Assess injured workers’ medical conditions and develop coordinated treatment plans with providers
• collaborate on individualized care plans
• coordinate services across departments and external providers
• analyze needs and recommend interventions
• prioritize needs, select providers, and adjust plans
• communicate with workers, medical professionals, employers, and insurers
• attend appointments as needed
• recommend job-duty modifications based on restrictions and functional assessments
• explain medical instructions
• advocate and educate throughout recovery and return to work
• maintain licenses and certifications
• prepare monthly progress reports and document time and activities.
