Social Worker — Discharge Planner

What you'll do

• Provide direct psychosocial intervention
• assess residents at admission, upon condition change, and annually
• create, review, and update care plans and progress notes
• coordinate visits with outside services
• attend and document resident council meetings
• support families coping with skilled nursing placement, illness, disability, and grief
• plan discharges with patients, families, and team members
• conduct staff in-service programs on psychosocial issues and patient rights
• supervise and guide Social Services Assistants.
