What you'll do

• Oversee daily Social Services department operations and supervise staff
• assess residents’ psychosocial, emotional, and social needs
• develop care plans with the interdisciplinary team
• coordinate admissions, transfers, discharges, and community referrals
• advocate for residents’ rights and regulatory compliance
• facilitate family meetings, care conferences, and grievance resolution
• maintain documentation
• coordinate with community agencies, physicians, and healthcare providers
• assist with advance directives, placement, and financial concerns.
