What you'll do

• Conduct outreach, engagement, and follow-up with members
• coordinate services and appointments, including reminders and transportation
• participate in health promotion activities
• share relevant information with the care manager and care team
• provide and track referrals
• assist members in obtaining community resources and social support
• participate in case conferences
• support assessment of unmet health-related resource needs.
