Case Manager — Lied Bridge

What you'll do

• Provide trauma-informed care related to residents’ housing needs
• maintain assigned client files
• arrange intake assessment meetings and case conferences
• conduct unit inspections and prepare reports
• meet monthly with transitional housing participants about compliance, budgeting, and permanent-housing plans
• maintain rosters and deposit records
• coordinate repairs
• support unit readiness and occupancy
• enter HMIS/CMIS data and compile monthly statistics.
