What you'll do

• Advocate for clients and address external barriers to treatment
• initiate case management at intake
• oversee client accounts and confirm insurance remains active during treatment
• provide ongoing case management
• arrange post-discharge resources as needed
• facilitate discharge planning for continuing care, housing, and sober support.
