Community Health Worker

Job overview

The Community Health Worker is an onsite role in Shelbyville, IN, with pay of $25 per hour. The position is funded by the Indiana Department of Health Rural Health Transformation Grant and supports the YMCA’s Prescribe the Y program and chronic disease prevention initiatives. The worker helps participants build healthy behaviors, engage with evidence-based programs, and overcome barriers to participation. By coordinating coaching, education, referrals, and follow-up, the role contributes to participant wellness and program outcomes.

What you'll do

• Coordinate participant intake, orientation, enrollment, and engagement
• provide individualized coaching on behavior change, goals, nutrition, physical activity, stress management, and healthy lifestyles
• facilitate chronic disease prevention programs and workshops
• monitor progress and follow up
• connect participants with YMCA programs, healthcare providers, and community resources
• maintain confidential records and program outcomes
• support recruitment, referrals, outreach, and promotion.

Why this role

Bachelor’s degree preferred or equivalent relevant experience may be considered; certification may be obtained within six months, and CPR, AED, and First Aid certification is required within 30 days.
