What you'll do

• Provide comprehensive primary care for acute, chronic, and preventive needs
• conduct health assessments, examinations, and diagnostic evaluations
• diagnose and treat conditions affecting seniors
• develop care plans with patients, caregivers, and interdisciplinary teams
• prescribe medications and manage treatment
• order, review, and interpret diagnostic testing
• educate patients and families
• coordinate with clinical professionals, social workers, case managers, and community resources
• participate in population health, chronic care management, transitions of care, and quality improvement
• maintain EHR/EMR documentation
• and mentor advanced practice providers and support staff. Requires an MD or DO, unrestricted state medical license, board certification or eligibility in Family Medicine, Internal Medicine, or Geriatrics, and an active DEA license with prescriptive authority.
