Home Care Intake Coordinator

The Home Care Intake Coordinator role is onsite in Bala-Cynwyd, PA, Monday through Friday from 9:00 a.m. to 5:00 p.m. Pay is $24–$26 per hour. Eminence Home Care’s mission is to support client dignity and independence at home through individualized care plans. The coordinator serves as an initial contact for prospective clients and families, gathers intake information, and works with clinical staff to determine appropriate services. The role helps make onboarding into home care programs organized and supportive.

What you'll do

• Conduct initial assessments and gather client and family information
• complete intake documentation accurately and on time
• collaborate with clinical staff to determine service levels
• communicate with clients, families, and the home care team
• follow regulations and company policies
• explain available services and support onboarding.
