Hospital Liaison Nurse

What you'll do

• Maintain relationships with hospital discharge planners, case managers, social workers, and other referral sources
• promote post-acute care services
• coordinate with facility leadership on admissions and satisfaction
• provide clinical onsite coordination with hospital networks
• develop data-informed census growth plans
• represent the organization at community and healthcare events
• report referral pipeline activity and growth metrics.
