The Community Habilitation Specialist — Per Diem role is onsite in Troy, NY. Pay is $20 per hour. The specialist helps people work toward goals in agreed service plans through habilitation, skill-building, respite, and related assistance. Service documentation, communication, and coordination with families and community resources support individualized care and progress.

What you'll do

• Provide direct care and services to individuals and families
• assist with goal-oriented activities based on service plans
• maintain knowledge of community resources and recreational activities
• participate in supervision, staff development, and committees
• communicate effectively with individuals and families
• create schedules
• complete service summaries, intake assessments, and discharge summaries.
