Case Manager

Job overview

The Case Manager role is onsite in Laconia, NH, with compensation up to $50,000 per year. Northeast Family Services provides mental health services to children, youth, and families. The Case Manager coordinates in-home and community-based services with clients, families, DCYF, and other treatment-team partners. The role supports treatment goals, family safety, and continuity of care through planning, coaching, documentation, and ongoing risk assessment.

What you'll do

• Maintain an assigned caseload and support implementation of in-home services
• collaborate with DCYF on referrals, treatment plans, meetings, and documentation
• prepare assessments and reports
• provide parent education, coaching, and supervised visitation
• report progress to courts as requested
• coordinate with schools and behavioral health providers
• assess client risk
• transport clients
• participate in an on-call rotation and provide early-evening availability.
