What you'll do

• Conduct initial and ongoing searches for absent parents and relatives
• participate in referral planning meetings
• support families in meeting children’s needs
• plan and facilitate permanency and assist relative and kinship caregivers
• maintain relationships with public and private agencies
• provide relevant information to case teams
• attend case planning conferences
• conduct home visits
• coordinate community resources and child placements
• complete documentation and reports
• coordinate service delivery with internal and external teams
• maintain confidentiality of youth, foster care, adoption, and foster parent records.
