What you'll do

• Direct and supervise Coordinators of Care and other clinical staff
• ensure care-plan reassessments and timely recertification packages
• evaluate services through record reviews, field visits, and case conferences
• communicate with staff and patients
• advocate for patients with third-party payers
• identify staffing and service needs
• support emergency disaster planning
• guide staff on difficult patient issues
• participate in hiring, training, and discipline
• coordinate conversion packets
• participate in an on-call coverage rotation.
