Foster-Adoptive Care Case Manager

The Foster-Adoptive Care Case Manager role is hybrid in Charleston, WV, with pay of $17–$19 per hour depending on experience and licensure. KVC West Virginia works to support the safety and stability of children and families. As a treatment-team member, the case manager supports foster and adoptive families through visits, advocacy, documentation, and coordination with community partners. The role contributes to child safety, stability, and well-being while meeting state and Medicaid standards, and includes a 24-hour on-call rotation for crisis support.

What you'll do

• Provide feedback on client progress as a treatment-team member
• conduct home visits at least twice monthly or as directed by the treatment plan
• perform monthly school visits
• attend multidisciplinary team and IEP meetings to advocate for children
• document to meet child-placing and Medicaid standards
• participate in a 24-hour on-call rotation
• facilitate supervised visitation
• maintain relationships with the Department of Human Services.

Why this role

Hybrid position with travel throughout the service area. A bachelor’s degree in a social-service-related field and at least two years of children and family services experience are required; social work licensure is preferred but not required.
