What you'll do

• Conduct ongoing clinical assessments to identify psychosocial, financial, environmental, and community resource needs
• develop and evaluate plans of care with patients and caregivers
• provide counseling and education as ordered and included in the plan of care
• document assessments, interventions, and communications
• collaborate with the interdisciplinary team
• participate in QAPI and in-service training
• maintain knowledge of eligibility standards.
