Hospice Intake Coordinator

What you'll do

• Receive professional referrals and obtain referral information
• enter referrals and patient information into the EMR
• receive patient and family inquiries
• coordinate and schedule initial visits
• communicate with referral sources, attending physicians, staff, and other agencies
• maintain communication through sign-on, consent, and admission assessment
• maintain accurate EMR documentation and referral statistics.
