Adult Case Manager

What you'll do

• Support adults with intellectual disabilities or autism and their families by advocating for choice and satisfaction with services and monitoring health and safety.
• Work with individuals and their teams to identify needs, develop care plans, initiate referrals, coordinate services, monitor progress, and evaluate service effectiveness.
