What you'll do

• Participate in admissions decisions and coordinate admission timing
• collaborate with clinical teams, clients, families, and healthcare providers
• document progress toward recovery goals and services provided
• educate and advocate for clients and families
• maintain a list of community resources
• assess client needs and barriers
• coordinate access to community services and transportation using personal or organization vehicles
• build relationships with service providers
• participate in treatment team meetings
• assist with discharge and community transitions.
