Hospice Care Team Coordinator

Job overview

The Hospice Care Team Coordinator role is based in Golden Valley, MN, with a Monday–Friday schedule from 8:00 a.m. to 5:00 p.m. and one remote weekend per month. Compensation is $25–$30 per hour, depending on experience. Superior Hospice serves patients and families throughout Minnesota with individualized end-of-life care focused on comfort, dignity, symptom management, and emotional and spiritual support. The coordinator connects referral partners, patients, families, and the hospice clinical team so referrals and admissions can move forward without unnecessary delays. The role gathers records, maintains accurate EMR information, communicates next steps, and follows up on missing information or barriers to care.

What you'll do

• Receive and review hospice referrals
• gather medical records and required documentation
• communicate with referral partners, patients, and families
• coordinate intake and admissions next steps with clinical staff
• maintain patient and referral information in the EMR
• manage scheduling and changing priorities
• provide updates
• and follow through on missing information and barriers that could delay care.
