Community Liaison

The Community Liaison role is based onsite in Durango, CO, with visits to community partners and clients’ homes throughout the service area. What you'll do

• Build and maintain referral relationships
• visit healthcare and senior-care referral sources
• represent the organization at networking events and health fairs
• develop marketing strategies
• follow up with partners and prospective clients
• assist with admissions and home check-ins
• support caregiver recruiting and new-hire orientation
• and track outreach and referral activity.
