What you'll do

• Review and monitor Individual Service Plans and their implementation
• review Therap documentation and maintain records in eBridge
• attend ISP, interagency, and DBHDD meetings as requested
• communicate with families and natural supports
• review medical records and required forms
• monitor provider training and compliance
• conduct monthly home visits and inspections
• support and supervise providers
• complete annual assessments and evaluations
• assist with host home provider recruitment and applications.
