What you'll do

• Conduct initial and ongoing comprehensive assessments
• determine admission and recertification eligibility
• develop, implement, and document plans of care
• educate patients and families on self-care and nutrition
• determine service scope and frequency
• provide preventive and rehabilitative nursing procedures
• follow infection-control precautions
• direct home health aides and LPN/LVNs
• document progress
• communicate changes to physicians and care team
• monitor cases for payer requirements.
