Home Health Registered Nurse — East/Central Territory

Job overview

The Home Health Registered Nurse role is onsite in Chicago, IL, serving the East/Central territory and surrounding communities; pay is $60–$120 per hour. The nurse supports Anuco Home Health Care’s Clinical Services team in providing in-home care that promotes recovery, safety, independence, and quality of life. The nurse assesses patients, coordinates individualized care, provides skilled nursing services, and documents care accurately. The role also supports patients and caregivers in managing health safely at home and coordinates with the interdisciplinary care team.

What you'll do

• Complete nursing and OASIS assessments
• develop and revise individualized plans of care
• provide skilled procedures, treatments, medication management, education, and preventive or rehabilitative care
• coordinate with physicians and interdisciplinary providers
• supervise assigned LPNs and Home Health Aides
• and complete timely Axxess documentation. Provide on-call, weekend, or holiday coverage when assigned.
