Case Manager

The Case Manager role is onsite in Homestead, FL. Pay is $24.52 per hour. Chapman Partnership case management serves residents and families, supporting their social and emotional needs and safe transitions between environments. The role exists to assess residents’ needs, connect them with services and community resources, and help them stabilize and regain independence. Case managers also support follow-up and discharge planning and maintain resident records and program reports.

What you'll do

• Assess residents and families for services and develop individual service and goal plans
• participate in case conferences
• connect residents with community agencies and resources
• provide outplacement and follow-up services
• maintain HMIS case records
• prepare statistical and other reports
• manage customer satisfaction surveys and analyze productivity reports
• assist with resident transportation needs and report vehicle maintenance needs.

Why this role
