The role exists to identify and assess the nutritional needs of assigned individuals, initiating interventions to maintain or improve their nutritional status. By collaborating with the interdisciplinary team and providing resident-centered care, the dietitian contributes to enhancing the overall quality of life for those in inpatient and outpatient settings.

What you'll do

• Provide nutrition assessments for admissions, quarterly and annual reviews, tube feedings, and significant changes.
• Create and update care plans for nursing staff.
• Conduct nutrition education, complete MDS documentation, and perform nutrition-focused physical exams.
• Participate in care conferences, clinical meetings, QAPI meetings, and perform kitchen inspections.
