What you'll do

• Coordinate and oversee direct-care patient services and clinical operations for the managed location. Assess patients and update care plans
• review referrals and determine home health needs and admission eligibility
• coordinate palliative care assessments
• supervise clinician assignments and care delivery
• guide clinicians on goals and plans of care
• monitor documentation and regulatory compliance
• oversee audits and billing timelines
• supervise and coach clinical staff.
