What you'll do

• Assess patients and develop individualized plans of care
• provide nursing interventions and evaluate outcomes
• monitor changes in condition and communicate with the care team
• educate patients and families about care, comfort, medications, and hospice resources
• collaborate with physicians and interdisciplinary team members
• maintain timely clinical documentation
• follow nursing standards, organizational policies, and regulations
• travel to patient locations throughout the assigned service area.

Why this role
