Clinical Nurse Manager

What you'll do

• Coordinate referrals and patient intake/admissions
• review clinical information and assess referral appropriateness for home health
• communicate with physicians, families, referral sources, and clinical teams
• maintain referral and admission records
• coordinate insurance verification and authorizations
• train new and existing staff
• prepare and maintain the on-call schedule and serve as on-call backup
• support utilization review and quality assurance activities
• coordinate staff meetings and participate in case conferences.
