The Registered Nurse (RN) — Street Medicine, Enhanced Care Management is a field-based role in Scotts Valley, CA, with limited office or clinic time. Pay is $45–$55 per hour. Wellness Equity Alliance delivers integrated, community-based care for populations most impacted by health inequities; this role supports its CalAIM Enhanced Care Management program. The RN provides direct clinical care, outreach, and care coordination to people experiencing unsheltered homelessness. Working with a multidisciplinary team and each member’s Lead Care Manager, the nurse helps deliver whole-person care where members live, coordinate transitions and referrals, and support continuity of care.
What you'll do
- Conduct field nursing assessments, phlebotomy, specimen collection, point-of-care testing, medication administration, and wound care within scope and orders
- triage and escalate acute concerns
- conduct outreach and document consent
- contribute clinical input to care plans
- coordinate with providers and care teams
- provide health education
- support transitions from hospitals and facilities
- connect members to community supports
- document services in the EHR and meet program requirements.

Why this role
