What you'll do

• Advocate for residents’ rights, needs, and preferences

• assist with admissions, discharge planning, and transitions of care
• address resident and family concerns
• connect residents and families with community resources
• collaborate with nursing, therapy, activities, and dietary teams.

Why this role
